LONG TERM DISABILITY INSURANCE

Automatic enrollment after 12 months of employment.

1. Eligibility - All permanent full-time employees who work at least 30 hours per week.  Insurance becomes effective on the first day of the month coinciding with or following the date you complete one year of service, provided you are actively at work on that day.  If you were not at work because of sickness or bodily injury, your insurance will not become effective until you return to active work for one full day.

2. Coverage - Upon completion of the elimination period (180 days of total disability) the maximum monthly benefit available is up to 60% of monthly earnings prior to the onset of the disability, with a maximum monthly benefit of $6,000 and a minimum monthly benefit of $100.

3. Premium - Paid by the Authority.
